1.  After you have completed the Mastery Test above, press the SUBMIT button below and save the file (Certificate.rtf) to our computer. You may need to allow pop-ups temporarily for the fole to download. Internet Explorer users, please watch for a yellow bar to appear at the top of your browser window.

2. Log into your Desire2Learn account. (If you have problems please contact Technical Support at 1-866-550-7637)

3.  Using the e-Mail tool in Desire2Learn (which is private) you are to e-mail each instructor your name, address, phone number, and external e-mail address (like Hotmail, Yahoo, gMail, etc.) for emergencies. You are to attach the Certificate of Completion to the e-mail as an attachment.

Please know that this form is required for each of your classes. Your instructor may also use this form to verify attendance and that you are prepared for learning online.
